By completing and submitting the form below you are hereby agreeing to:

Release all production footage that you recorded during the Hold My Hand in the Water music video shoot to Broken Music including stills and video footage.
Allow Broken Music to record your image in the process of filmmaking for potential ‘behind the scenes’ or ‘making of’ the Hold My Hand in the Water music video releases or productions.
Not release any photos or media associated with the Hold My Hand in the Water music project in any public forums, social media or online, until after the official release date of the music video.
Release Broken Music from any and all future claims regarding the Hold My Hand in the Water music video production and distribution.
